Every student-athlete must complete the UIL paperwork online and complete a Medical history form paper every year. Every student must have a physical on file before participation and renewed when entering 7th,9th, and 11th grade before participating in athletics.  If an athlete needs a physical packet, please print one off from the link below or come by the field house athletic training room.  A completed Medical History form will need to accompany the packet – Medical History Form

Livingston  Athletic Training program and a network of sports medicine providers at Dedicated Orthopedic Center have combined to better serve your high school athletics program. By joining forces, we have created a team approach to athletic injury prevention, treatment, and the care and rehabilitation of your athletes. Here at Livingston Independent School District, you have 2 full-time athletic trainers who strive to give athletes the best health care possible.

  

Athletic Training Room Hours 

Morning Treatments 6:30 AM – 7:05 AM Off-season Athletes By Appointment

 

After School Treatments 3:30 PM- 4:00 PM 

Closed for Lunch 12:00 PM – 1:00 PM 

All athletes are required to wear athletic clothing and follow posted rules to receive treatment. 

Off-season sports athletes are required to inform the Staff Athletic Trainers the day BEFORE showing up for treatments.
