GEAR UP Volunteer Form

GEAR UP is excited for a new school year full of college tours and field trips! We would love to have parent chaperones for our GEAR UP trips this year. If you would like to volunteer to attend a GEAR UP-sponsored trip, please fill out the form below and return it to the GEAR UP Coordinator Julie Kuylen with a copy of your driver's license. (The second page only needs a date and signature.)
